Consultant Benefits

OtterBase is dedicated to providing employee benefits that not only match industry standards, but exceed them. Benefit packages are custom design to meet each contractor’s individual needs. Certain benefits require employee contributions. Please refer to your Employment Agreement and Benefit Exhibit for a more detailed explanation of the benefit packages available per your specific Employment Agreement.

Medical Insurance with Prescription Drug Benefit:
Medical and Prescription Drug Benefits are available to eligible, full-time employees under the OtterBase Health Plan. The plan provides comprehensive health insurance as more fully described in the plan benefit book. Benefits for employees are available through bi-weekly employee contribution as established by the plan. OtterBase’s Section 125 plan allows employee contributions to be taken on a pre-tax basis. Employee bi-weekly contributions may be adjusted when deemed necessary. Coverage is effective the first of the month following the date of hire if eligibility requirements are met.

Vision Discount Program:
The Aetna Vision One Discount Program offers you and your family savings on many eye care products, including eye exams, eyeglasses, LASIK surgery, contact lenses, nonprescription sunglasses, contact lens solutions and other eye care accessories.

Choose from a large network of optical centers nationwide including Sears, JCPenney, Target and participating Pearle Vision centers, as well as independent optometrists and ophthalmologists.

Employer Sponsored Voluntary Long Term Disability:
This is an individual policy with discounted premiums, which are lower than what can be purchased directly from the insurance company. The benefits can be custom designed to meet your specific needs and this plan is portable. Your premiums are paid through convenient payroll deductions on a post-tax basis.

 

Employer Sponsored Voluntary Life:
This is an individual plan and a wide range of products are available. This benefit can be custom-designed to meet your specific needs and this plan is portable. Your premiums are paid through convenient payroll deductions on a post-tax basis.

 

OtterBase 401(k) Retirement Savings Program:
OtterBase’s 401(k) plan is administered by SmithBarney. This flexible plan allows employees the opportunity to contribute up to $15,500 per year through convenient pre-tax payroll deductions. Your contribution may be invested in stocks, bonds, mutual funds and/or money funds. Our Plan Administrator can assist OtterBase employees with rollovers and other financial/investment issues.
OtterBase does not have a matching contribution plan at this time.

OtterBase Section 125 Plan:
Section 125 Plan allows OtterBase to take your employee contributions for the medical and dental plans on a pre-tax basis.

 

OtterBase Medical Flexible Spending Plan:
Health Care Flexible Spending Account allows employees contribute to a Flexible Spending Account Plan (FSA) on a pre-tax basis through convenient payroll deductions. Health Care FSA contributions may be used to for eligible health expenses that are not covered by the medical and dental plan. Examples of such expenses would be office visits co-pays, prescription co-pays, and eyeglasses.

Maximum Annual Contribution: $3,000

 

OtterBase Dependent Care Flexible Spending Plan:
Dependent Care Flexible Spending Account allows employees contribute to a Flexible Spending Account Plan (FSA) on a pre-tax basis through convenient payroll deductions. Dependent Care FSA contributions may be used to cover eligible child care (daycare) expenses.

Plan Year: January 1 through December 31
Maximum Annual Contribution: $5,000 (married & files a joint return) or $2,500 (married & files a separate return)

 

Paid Holiday and Personal Days:
Holiday Benefit:
6 holidays – New Year’s Day, Memorial Day, 4th of July, Labor Day, Thanksgiving Day, and Christmas Day.

Personal Time Off Benefit:
Personal time is accrued, not to exceed the maximum number of days noted in the Employment Agreement each year.

Employee Referral Bonus Program:
All OtterBase contractors are eligible for a $200 bonus if a referral is placed by OtterBase and stays in the position for at least 90 days.
